If you are U.S. citizens or residents, live in the U.S. and would like to receive more information about Commonwealth Adoptions' programs, please complete and submit the following form to us via email. If you would like the opportunity to discuss your adoption with an Agency Associate, please be sure to include a day time phone number.
Note: This questionnaire is intended solely for the use of U.S. families residing in the U.S, and U.S. military families stationed abroad. At this point, we are unable to serve any foreign families who are residing in a foreign country, regardless if U.S. citizens or not.
