CAI will not share or give out your personal information except to companies that are directly related to your adoption.
Filling out this form requires approximately 20 to 30 minutes. If you are contacting us for the first time or simply want to get some information about our adoption programs, we encourage you to request a brochure instead. We will mail you a comprehensive information packet and arrange an Adoptive Consultant to email you or call you in about 2 weeks. Currently, Commonwealth Adoptions chooses to serve U.S families only, either from within the U.S. or U.S. military families stationed abroad. If you do not belong to this category, you may want to check for additional information from our Foreign Families page.
Step 1. Fill out this multi-page application. You will be prompted automatically if required fields are not filled in after each page. After you have completed the application, review your submitted data and submit.
Step 2. If you have been communicating with one of our Adoption Consultants, please let us know by clicking on his/her name on the list of Adoption consultants. Otherwise, we will assign an Adoption Consultant to follow up with you on your application.
Step 3. Mail us the $400 application fee for various adoption programs. Please include a photo with your application fee.
